LCBDD Certificate of Agreement for Early Intervention Lending Library
I, the undersigned individual/parent/guardian/caregiver, request the loan of items from the Lorain County Board of Developmental Disabilities for use by the student/consumer listed on this account.
I, the undersigned, agree to use the item for its intended function for the named student/consumer and to properly maintain the item. If damage occurs, I accept responsibility to repair the item to its original condition when loaned or to replace it with a comparable item if repair is not possible. I understand the Agency assumes no responsibility/liability for my use of the item.
I further agree that the loan start date for the named item will be for 90 days or for the term indicated at check-out and the item will be returned to the Lorain County Board of DD at the end of the loan term.
